Please list potential drivers for your cart rental (including their driver's license numbers & expiration dates).

List no more than 3 names (full name as it appears on DL) along with DL numbers & expiration dates. Please list the primary driver first.

Please note specific reasons as to why you would like to rent a golf cart this summer.

Note: This reason could be for transportation of an older volunteer, handicapped student, or even just to leisurely ride around campus.

I have reviewed the AU golf cart policies above and agree to all listed.

I will pay $250 upon confirmation from Crossroads for 1 golf cart/group.

My group will be responsible for covering the cost of any damages to our assigned golf cart as they are assessed each week.

I will make sure to not have more than 5 people on the golf cart at one time.

No one will be standing on a moving golf cart with any of our drivers. The back footplate cannot hold that weight without breaking/bending.

I will adhere to all rules/regulations implemented by the host university & Crossroads Summer Camp regarding the use of our group's golf cart.
